EVENTS TERMS & CONDITIONS
Indemnity: I understand that while every reasonable precaution will be undertaken to ensure the participants protection, I hereby release CYM Perth, authorised staff and volunteers from any and all liability in the event of any injury, accident, misfortune, damage or loss that may occur to the participant and/or the participants property while present at this event. Further, I indemnify CYM Perth, authorised staff and volunteers from and against all loss, including legal expenses, connected with, or arising from any claims or demands in relation to the participants attendance at this event.
Medical Treatment Consent: I give permission for CYM Perth, authorised staff and volunteers to obtain emergency medical, hospital or ambulance assistance and/or treatment for the participant at any time they consider necessary. I acknowledge that the participant will be liable for any medical, hospital or ambulance expense incurred for the participants treatment and agree to pay those expenses. Further, I give permission for Paracetamol to be administered to the participant at the discretion of CYM Staff or designated First Aid officers.
Involvement Consent: I agree and understand that CYM Perth reserves the right to exercise discretion to refuse to register any person upon medical and/or other grounds without providing a detailed reason for doing so. This includes inappropriate behavior, being under the influence of drugs or alcohol, or being in possession of drugs or alcohol.
Privacy Declaration: I understand that CYM Perth may collect information about the participant for the purpose of providing promotional material, and that they will not pass the information on to any other organisation. I consent to the participants details being used by CYM Perth for the promotion of future events and resources via post, phone, email and SMS, I can elect to have the participants details removed from their database at any time. This event may be captured in photographs, video and audio. CYM Perth reserves the right to use this material for promotional purposes or other resources.
